The pharmacology problem

An injected peptide bypasses the digestive system entirely. Taken by mouth it faces stomach acid, digestive enzymes and poor intestinal absorption — which is why oral peptide delivery is a genuinely hard problem that took years and specific formulation technology to solve.

Oral semaglutide solves it with an absorption enhancer, and the resulting product requires administration in the morning on an empty stomach with no more than four ounces of water and a thirty-minute wait. That regimen exists because the underlying problem is real.

Orforglipron sidesteps it by being a small molecule rather than a peptide, which is why it has no food or water restrictions.

A compounded orally disintegrating tirzepatide tablet inherits neither solution. How much drug reaches the bloodstream is the entire question, and for these preparations it is unpublished.

What sublingual and ODT actually mean

Sublingual administration aims to absorb through the mucosa under the tongue, bypassing first-pass metabolism. That works well for some small molecules. Peptides are large, and mucosal absorption of a molecule the size of tirzepatide is not something you can assume from the route alone.

ODT means the tablet disintegrates in the mouth. That is a convenience feature about how the dose is taken; it says nothing by itself about where absorption occurs or how much of it happens.

If needles are the barrier
