Flat-Rate GLP-1 Pricing: What It Means and When It Saves Money

What flat rate actually means

One monthly price, unchanged whether you are on 2.5 mg or 15 mg. No dose surcharge, no tier jump at 7.5 mg, no repricing at refill.

It sounds like a minor billing detail. Over a year at a maintenance dose it is frequently the largest single variable in what you pay — larger than the advertised monthly figure that most comparisons rank on.

The arithmetic, worked

Brand Zepbound direct: $299 at 2.5 mg, $399 at 5 mg, $449 from 7.5 mg upward, with the rate contingent on refilling within 45 days. Following the label's titration, roughly $5,088 across a year.

A flat-rate compounded plan at $186: $2,232 across the same year.

The $2,856 gap comes entirely from structure. Neither advertised monthly figure predicts it. These are different products with different regulatory status, so this is not a like-for-like recommendation — the mechanism is the point.

Who holds price across doses

Captured by us: NexLife, across all six programmes, confirmed on its own plan pages 11 July 2026. Compounded tirzepatide at $186 on twelve months, $215 month-to-month, flat at every covered dose, no membership fee.

Provider-reported: Oak Longevity, TrimRx, Mochi Health, Eden and Enhance.MD describe flat medication pricing, though several bill a separate membership on top — Mochi at $79 a month, Eden at $99. Found describes medication as included and flat at all doses on prepaid plans.

Escalates: Shed states its compounded programmes increase at higher doses. MEDVi is reported to reach $499 at maintenance doses. Both brand manufacturers tier by dose.

When flat rate is the wrong choice

If you maintain at a low dose. A tiered programme's entry rate can genuinely beat a flat rate for someone who stays at 5 mg, and there are real clinical reasons a prescriber might favour the lowest effective dose — the lean-mass evidence and the osteoarthritis threshold data both point that way.

The problem is that you cannot know your maintenance dose at signup. That asymmetry is the argument for comparing at 10 mg: if you end up lower, you have overpaid modestly; if you end up higher on a tiered plan, you have overpaid substantially.

The inverse trap: dose caps

Some programmes cap the maximum dose rather than charging for it. One compounded semaglutide programme in our dataset is capped at 0.6 mg against the 2.4 mg used in the STEP trials, at $199 a month, with the full-titration version at $279.

A capped programme is a lower-dose treatment with a correspondingly smaller expected effect, not a discounted version of the same one. It should never appear in the same ranking as full-titration programmes, and on this site it does not.

What to get in writing

1. The price at 2.5, 5, 10 and 15 mg, separately.
2. Whether a membership is required to receive medication.
3. Whether the advertised rate is introductory, and what the ongoing rate is.
4. What is refunded if you stop mid-term on a prepaid plan.

A provider that answers all four plainly can be compared accurately — which is generally to the advantage of providers with nothing to hide.
